Whether this be current vaccinations/immunisations or recent diagnosis of medical conditions such as ADHD, asthma, etc. We require action plans and instructions for medication if needed.

Bills are up to date:

We cannot re-enrol without your balance being up to date. Please double check your statements via the HOME app to ensure you are on track.
